  Ischemic strokes of the brain are often treated with clot busting drugs - they dissolve the clots molecule by molecule, not in big chunks - dissolving plaque would work the same way. Think of plaque as a gobbstopper jawbreaker - your saliva slowly dissolves the outer layers one by one until you get to the center surprise - the saliva can't pass thru the outer layers to crack the piece of candy inside - dissolving plaque and clots works the same way, only the outer layer exposed to the drug can be dissolved, then the next layer, and so on.
